  • You complete a brief daily check-in through a simple app or phone questionnaire — it takes less than 2 minutes
  • Your responses are reviewed by your care team each day
  • If your pain spikes, medication adherence drops, or symptoms worsen — your team reaches out right away
  • Your physician uses the data to adjust your therapy or treatment plan without waiting for your next scheduled visit
  • Monthly summaries are shared with all members of your care team to keep everyone aligned

RTM vs. RPM — What's the Difference?

RPM

Tracks physiological data — blood pressure, weight, oxygen, blood sugar — using connected devices.

RTM

Tracks therapeutic data — pain levels, medication adherence, and symptom response — using software and questionnaires.

Many patients benefit from both programs together. Ask your care coordinator which is right for you.

Covered by Medicare

RTM is covered under Medicare as of 2022. Eligible patients can receive monitoring, data review, and clinical interactions at little to no cost under Part B.
