Yes — most of our care programs, including Remote Patient Monitoring and Chronic Care Management, are covered by Medicare Part B at no additional cost to patients. Many private insurance plans also cover these services. Our team will verify your coverage before you enroll so there are no surprises.

Not at all. Our devices are designed to be as simple as possible — most require just one button press. We deliver your devices to your home, set everything up for you, and walk you through exactly how to use them. If you ever have trouble, your care team is just a phone call away.

Your care team monitors your readings every day. If something looks unusual — for example, if your blood pressure is unusually high or your weight changes suddenly — your monitoring team will contact you right away to check in. If needed, they'll coordinate with your doctor to adjust your care plan.

No. If you don't have a primary care provider, we can serve as your primary doctor through our Home-Based Primary Care program. We also offer telehealth-only visits and temporary care for patients recovering from illness or surgery.

Your readings are reviewed daily by your monitoring team. Your care coordinator will reach out at least once a month for a personal check-in. Depending on your program, you may also have regular home visits, telehealth appointments, and on-demand access to your care team whenever you have a question or concern.

Absolutely. All data transmitted from your devices and all communications with your care team are fully HIPAA-compliant and protected by secure encryption. We never sell your data, and only your care team and authorized providers can access your health information.

Yes — and we encourage it. With your permission, we can include a family member or designated caregiver in your care communications, home visits, and monthly check-ins. Having a support person involved often leads to better health outcomes.
