Joji Jacob, LCSW, is a Licensed Clinical Social Worker with more than 22 years of experience in palliative care, hospice, and care coordination. She is dedicated to helping patients and families navigate serious and chronic illnesses through compassionate counseling, advance care planning, and personalized support that enhances quality of life.

Throughout her career, Joji has collaborated with multidisciplinary healthcare teams across hospitals, home health agencies, hospice organizations, and community-based care settings. She has extensive experience in goals-of-care discussions, psychosocial assessments, care planning, and connecting patients with essential community resources to ensure holistic, patient-centered care.

Joji earned her Master of Social Welfare (MSW) from the University of California, Los Angeles (UCLA) and her Bachelor of Arts in Psychology and Social Welfare from the University of California, Berkeley. She is a Licensed Clinical Social Worker (LCSW) in California and a California BBS Certified Supervisor.
