Living with a chronic condition means you need more than just an annual appointment. With CCM, your care team is in your corner every month — proactively managing your health so you can focus on living your life.

Who CCM Is For

Diabetes Heart Disease Hypertension COPD Chronic Kidney Disease Arthritis Depression Alzheimer's

You qualify for CCM if you have two or more chronic conditions expected to last at least 12 months. Most Medicare patients are eligible.

What Happens Each Month

  • A dedicated care coordinator calls to check in on how you're feeling
  • Your medications are reviewed and updated if needed
  • Specialist appointments are scheduled and coordinated for you
  • Your care plan is updated based on what's changed
  • You have 24/7 access to your care team for urgent questions

Covered by Medicare

CCM is a Medicare-covered service under Part B. Patients with two or more qualifying chronic conditions are typically eligible with little to no out-of-pocket cost.
